Skill Acquired: Patient History Taking


Taking a patient's history is very essential to make a diagnosis, together with physical examinations. From a full patient's history, we could decrease the probability of wrong diagnosis.

What to do?
1)Invite the patient into your office, greet him, introduce yourself, address his name appropriately
2)"I understand that there is some problem with your health, May i know it more?"
3) identifying the Chief Complaint and its duration, how the illness begin,
4) Reassurance is the symptom had happened in his lifetime be4
What and where is the symtom?
Quantity and Quality of the symptom
Aggrevating/Relieving factor
Associating factor
Patient's belief

Other History:
Previous individual medical history
Family history related to symptom
Medication history
Social history
Sex/ Menstrual History
